JFS care managers:
Conduct in-home assessments of each person's strengths and needs
Develop a comprehensive, individualized care plan designed to address those needs
Maintain regular contact with family members, near or far, when appropriate
Coordinate and monitor in-home services
Provide healthcare advocacy
Assist with long-term care planning
Give information, referrals, and assistance connecting to additional community resources
Our care managers are also available to provide consultation services by creating a proactive plan for individuals, couples, and families in an effort to ease the stress of aging and the unknown territory that comes with it.
The Aging Care & Connections team helps older adults maintain as much independence as possible, while preparing for unknowns that may occur with age. We work alongside our clients to develop a plan of care that ensures opportunities for self-sufficiency, support, and dignity. We make connections to local and national resources for home care, healthcare, mental health support, nursing, transportation, and safety as needed.
