What’s on the Boulder horizon
Partnerships play a significant role in the work that we do and the success of the organization. Each day, we are excited and humbled to have the opportunity to help those who rely on our programs and services, and we recognize our work can only happen because of the support we receive. Spend a few minutes reading about some highlights we want to share about two recent grants.
Boulder Community Health Foundation: Community Collaboration Grant
This grant, which began on January 1, is with the Boulder Community Health Foundation and covers care management for individuals in Boulder County.
JFS Boulder’s Care Management program aims to address critical issues affecting older adults in Boulder, including social isolation, the desire for aging in place, chronic health conditions and access to community resources. The program is designed to provide comprehensive support and services tailored to the complex needs of this population.
With the Community Collaboration Grant, our objectives include:
Service expansion: Increase the number of unduplicated individuals served from 21 to 35.
Comprehensive assessments: Conduct thorough benefits and financial assessments to ensure participants receive tailored support.
Family engagement: Facilitate regular family meetings and provide access to Caregiver Support Groups for enhanced decision-making and support.
Wraparound supports: Offer emergency assistance, social engagement through the Friendly Visitors program, healthcare coordination, transportation, meal deliveries, and holiday meal distributions.
Our long-term goals include:
Enhanced wellbeing: Improve social and emotional support through ongoing community engagement.
Continued care: Maintain effective care management and coordination to boost health outcomes.
Sustainable services: Refine the service delivery model for long-term sustainability and adaptability to community needs.
City of Boulder Health Equity Fund
The grant with the City of Boulder provides low-cost mental health services to residents of the City of Boulder. While the contract period began on January 1, contract execution is pending.
With the City of Boulder Health Equity Fund grant, our goals include:
Expanding mental health counseling and treatment: JFS Boulder will provide evidence-based mental health counseling. Using therapeutic approaches, JFS Boulder’s licensed therapists will address the mental health needs of individuals and families facing significant life challenges. These approaches are proven to effectively reduce symptoms of anxiety, depression, trauma, and other mental health issues, enabling clients to achieve better overall health and wellbeing.
Providing wellness and support groups: In addition to individual therapy, JFS Boulder will provide wellness and support groups. These groups will also use evidence-based approaches and serve as a supportive environment where participants can process shared experiences, learn coping strategies, and build resilience.
