Jay and Rose Phillips Aging Care & Connections (ACC) provides a variety of services and supports to help older adults live independently. The primary focus is to help older adults remain as independent as possible for as long as possible in the environment of their choosing.

Based on our principles of inclusion, compassion, and social justice, JFS is committed to providing vital services to all older adults regardless of their faith, race, national origin, gender identification, or sexual orientation. The experienced ACC team provides consistent care, eases transitions, and creates a vibrant support team for older adults and their families. We work to create a comprehensive care plan that meets each individual’s needs in a way that preserves dignity and maintains quality of life. 

Many services are offered on a sliding-scale fee basis.

Working with the Colorado Health Institute, the ACC team conducted a community-wide needs assessment to identify gaps in the available programs and services for older adults. The goal was to use the assessment findings to build the capacity to launch new programs to meet those needs. Read the full assessment report.

Care Management & Consultation

Our care managers are gerontology experts, committed to providing excellent service delivery and care coordination that maintains the dignity and achieves the goals of older adults served.

Counseling

Older adults and their family members can often benefit from compassionate counseling to cope with life’s transitions and other issues that come with aging. 

In-Home Services

JFS offers multiple in-home services for older adults. Our homemakers provide housekeeping assistance in a responsible and considerate manner. We deliver hot or frozen meals to homebound adults five days per week through our Kosher Meals on Wheels program. Friendly Visitor volunteers donate their time to visit with isolated older adults and enjoy a variety of activities together.

Community-based Programs

This program coordinates with city staff and other local senior service organizations with the intent of enriching the physical and emotional well-being of older adults by providing wellness, lunch, recreational, and social engagement opportunities.
