Care Management and Long Distance Care Management

Boulder JFS creates an effective support system for seniors that helps to meet their individual needs. JFS can help alleviate the caregiving burden often felt by family members or friends, whether they live near or far.

The Boulder JFS care manager will:

• Design an individualized
  care plan		 • Respond to client's changing
  needs
• Connect clients to JFS
  and/or community services		 • Provide peace of mind to clients
  and their loved ones
• Work with local or distant
  family members and/or
  friends
